2020 COVID-19 Policies and Procedures

These are Public Health Principles for each of the following steps. Please comply with them to protect other patients, Bison Vision Center staff and YOURSELF.

Before scheduling please ask yourself or the patient the following questions:

  1. Have you traveled outside Medford in the past two weeks? Y N
  2. Have you traveled outside Jackson County in the past two weeks? Y N
  3. Have you maintained isolation and quarantine for two full weeks? Y N
  4. Do you routinely use a facemask when out of your vehicle for day to day activities? Y N
  5. Have you had a cough or non-seasonal congestion in the past two weeks? Y N
  6. Have you had a fever above 100 degrees in the past two weeks? Y N
  7. Have you had contact with anyone who has tested positive for COVID-19? Y N
  8. Have felt “like yourself” for the past two weeks? Y N

You may be asked these questions repeatedly during our scheduling and examination services.

PLEASE NOTIFY US OF ANY ALLERGIES TO ALCOHOL, SANITIZERS, SURGICAL OR PAPER TAPE WHEN SCHEDULING.

When you schedule, please leave a contact number for VOICE, Text and e-mail. PLEASE SPEAK CLEARLY.

YOU ARE REQUIRED TO PROVIDE AND WEAR YOUR OWN FACE MASK AND IF YOU WISH, GLOVES.

When you arrive, please call to let us know you are here. Please verify your call-back number for VOICE and TEXT messages and for your e-mail address in voicemail or when you are asked for it by staff. Please speak clearly. YOU WILL BE ASKED TO WAIT IN YOUR VEHICLE.

Have your identification and insurance paperwork out and ready for verification.

If you are a new patient, YOU MUST FILL OUT ALL the patient forms including COVID-19 link forms on our website before arriving at the office for your appointment. If you are a returning patient, you are asked to update your contact and insurance information.

PATIENT WILL BE SEEN IN THE ORDER ON OUR SCHEDULE, WITH WORK IN EMERGENCIES ACCOMMODATE AT THE EARLIEST POSSIBLE TIME.

PLEASE MAINTAIN 6′ SPACING except when in the clinical setting.

PERSONAL FACE MASKS ARE REQUIRED AT ALL TIMES.

ONLY THE PATIENT AND ONE ACCOMPANYING PARENT, GUARDIAN OR TRANSLATOR WILL BE ALLOWED INTO THE OFFICE FOR THE APPOINTMENT. BROTHERS AND SISTERS MUST REMAIN IN THE VEHICLE UNTIL THEIR TURN.

DISPENSING OF EYEWEAR AND CONTACT LENSES WILL BE IN OUR SOUTH PATIO.

All equipment and rooms are disinfected between patients.

You MUST PROVIDE AND WEAR you own mask.

Doctor Smith and Cindy will wear Personal Protective Equipment as appropriate.

IF BISON VISION CENTER BECOMES CONTAMINATED BY COVID-19 AND NEEDS TO BE QUARANTINED/CLOSED, WE COULD HAVE EXPOSED OVER 200 PATIENTS BEFORE KNOWING THAT WE ARE CONTAMINATED.

Are we over-reacting? We will probably never know specifically if these policies and public health precautions had the desired effect. Looking at other cities, states and regions, we see the results of early lockdown, late lockdown and no lockdown. If I am wrong, I prefer to be on the side of CAUTION.

We must work together to protect my practice family from this unpredictable and too often deadly disease.
